Whether it’s short-term rehabilitation after hospitalization or the health journey is long-term requiring skilled nursing care, the focus is on restoring health so patients can live their best lives. Each patient works with a team of healthcare experts and professionals, including the medical director and other physicians, to develop a personalized plan of treatment to get back to the highest level of function. Together with their families and other advocates for their care, patients outline their goals and desires in individualized care plans.
